How to put claims in for Cancers that may be covered under Presumptive Legislation

The recent passage of the Presumptive Legislation to cover volunteer firefighters will now allow claims to be put in. I am supplying forms that can be filled out to start the process. The first form will be a WSIB Form 6 it is an actual WSIB report form for a worker. This should be filled out to as much information that the individual has. The second will be either the Worker Form or Deceased Worker Form. Please fill this out to the best of your ability, it is always best to include  work histories and possible exposures while firefighting. The length of service in total should be included.  If the person was at a large chemical incident then the date and if possible what the chemical was should be included example Hagersville Tire Fire.

Please fax both the filled out documents to the contact information on the Worker or Deceased worker form it will be to Cindy Price at WSIB. If you or a family member have previously put in a claim and it has either been denied or is still pending please contact the adjudicator or Cindy to make sure it is in the review process. They have assured us that all denied claims will be re-evaluated under the new presumptive legislation. 

The cancers that are currently under presumptive are:

Kidney Cancer-20 years
Colorectal Cancer -10 Years(diagnosed prior to 61st Birthday)
Non-Hodgkin's Lymphoma-20 years
Leukemia(certain types)-15 Years
Ureter Cancer-15 years
Esophageal cancer-25 years
Heart attack within 24 hours of fighting a fire or participating in training exercise involving a simulated fire emergency.

If you however do not meet this criteria by either not meeting the specific time periods or your cancer is not one of the types you may still put in a claim but will need to be very specific in details about calls and possible exposures. These claims will not be under presumptive and will be adjudicated individually as per the WSIB guidelines.
